How to Apply

We’re thrilled to welcome your family to Florida Diabetes Camp’s 2025 programs! Whether your child is joining us for a Summer Camp or Weekend Session, we’re here to make the registration process smooth and simple.

Each camper, friend, or sibling attending any program must have their own application completed by a parent or guardian. 

Applications are processed on a first-come, first-served basis, so we encourage early submission to secure your spot. A $25 deposit is required at the time of application to confirm your camper’s registration.

How to Apply

Our online application is straightforward to complete if you have all the necessary information ready. Follow these steps to apply:

  1. Visit www.floridadiabetescamp.org.
  2. Click on Camper Application in the top right corner.
  3. Log in or create an account:
    • Returning Families: Use the email address from prior registrations.
    • New Families: Create an account to start your application.
  4. Enter or confirm your camper’s information and follow the on-screen instructions.
  5. Select the desired sessions and complete all required sections.
  6. Submit your application along with the $25 deposit. To register additional campers, click Begin New Application after completing the first.
  7. You will receive a confirmation email once your application and deposit have been submitted.

Helpful Information to Have on Hand

To ensure a smooth application process, please gather the following information and documents beforehand:

Camper and Guardian Information
  • Camper’s full name, date of birth, grade, and school
  • Guardian’s contact details (email and phone)
Emergency Contacts
  • Two alternate contacts: Names, relationships, and phone numbers
Medical and Diabetes Details
  • Endocrinologist’s contact information
  • Insulin delivery method (pump or injections) and insulin types
  • Most recent A1C value and date
Documents to Upload with Your Application
  • Recent photo of the camper
Additional Forms (Required After Submitting Your Application)

After submitting your application, the following forms and documents will be required to complete your registration:

  • Health History Form
  • Physician Exam Form (completed by your child’s endocrinologist – summer camps only)
  • Copies of the camper’s immunization record
  • Copies of the camper’s insurance card (front and back)
  • Supporting documentation to support any requests for financial aid

Financial Assistance

At Florida Diabetes Camp, we believe every child with type 1 diabetes should have the chance to experience the fun, education, and sense of belonging our camps provide—regardless of their family’s financial situation.

To honor this belief, we offer Camperships, a financial assistance program designed to support families in need. Special Camperships are also available for newly diagnosed campers, ensuring they can benefit from our programs during a crucial time of adjustment.

Our Campership Eligibility Policy outlines the requirements and application process, making it simple for families to seek support and join our thriving FDC community.

I. Primary Campership Eligibility Criteria

Families may qualify for a full Campership if they meet any of the following criteria and provide the necessary documentation:

  • Income-Based Eligibility: Household income at or below 150% of the Federal Poverty Line (FPL).
  • Medicaid/Medicare: Proof of Medicaid or Medicare eligibility.
  • Free School Meal Programs: Proof of eligibility for Free School Breakfast and Lunch programs.
  • SNAP/TANF: Enrollment in the Supplemental Nutrition Assistance Program (SNAP) or Temporary Assistance for Needy Families (TANF).

Families may qualify for a partial Campership if they meet these criteria and provide supporting documents:

  • Income-Based Eligibility: Household income at or below 250% of the Federal Poverty Line (FPL).
  • Reduced Fee School Meal Programs: Proof of eligibility for Reduced Fee School Breakfast and Lunch programs.
II. Secondary Campership Eligibility Criteria: Extenuating Financial Circumstances

Families who don’t meet the primary criteria may still qualify for assistance based on extenuating financial hardships. In these cases, families can submit a written explanation detailing their circumstances. Secondary considerations may include:

  • A recent job loss or significant reduction in household income.
    Multiple children with T1D or substantial out-of-pocket medical expenses.
    Referrals from healthcare professionals such as doctors, social workers, or therapists.
III. Newly Diagnosed Campers

Campers diagnosed with type 1 diabetes within the year before their camp session are eligible for a full Campership. The specific cutoff date will be included in the financial assistance application and updated annually.

Requests for financial assistance can be submitted right in the camper application or through the form on our Camperships Page.
